This Notice of Privacy Practices is being provided to you as a requirement of federal law, the Health Insurance Portability and Accountability Act (HIPAA).

Your “protected health information” (PHI) refers to the information contained in your medical record regarding your physical or mental health. This also includes demographic information used to identify you. This Notice of Privacy Practices also describes your rights to access and control your PHI.
