We will mail the required forms for you to complete prior to your visit. If you cannot complete these prior to arriving, please plan to arrive 15-30 minutes early to do so at our offices. For your convenience we have also uploaded these documents and you will find them at the bottom of the page.
If you’ve had previous treatments or scans relating to your injury, compile them prior to your appointment and bring them with you or request that they be transferred to our offices.
If you do not have insurance or are unsure of your coverage for this appointment, please call our billing department at 515.537.2860, who are ready to help answer any questions you may have.
It is always a good idea to bring a list of all medications you are currently taking, along with a list of any medical conditions you might have. Please note, there is not a cafeteria at DMOS, so if you require frequent nourishment, remember to bring a snack.
If you are traveling from outside Des Moines and need to book a hotel stay before your appointment, or while a family member is having an operation, please visit hotels.com for rates and availability.
If you have a workers’ compensation claim or other liability claim, please make sure your employer/insured or their workers’ compensation carrier has provided written authorization of this visit. If you report the injury as a workers’ compensation claim during your visit and your appointment was not preauthorized, you will be responsible for the associated fees.
If you are unable to keep your scheduled appointment, please notify us so that we may use the time for other patients. Note that appointment times may not be the exact time you see a physician. We will likely need to prepare for your exam with the doctor by completing your registration, taking needed scans, reviewing your medical history, etc. We value your time and will always strive to make your visit with us as efficient as possible while also providing the highest level of care.
We require that anyone under the age of 18 be accompanied by a parent or legal guardian to the first appointment. For all other visits, all patients under the age of 16 years must be accompanied by an adult in order to be seen. If you, as the parent or legal guardian, are unable to accompany your child for a follow-up appointment, we will accept a written note with your signature allowing a friend or family member to authorize care for your child.
